Alien Registration

A Certificate of Alien Registration serves as confirmation of your identity, address and personal status. Every foreign resident (diplomats excepted)is required to apply for a Certificate of Alien Registration at a city office. For those under 16 years of age, someone who is over the age of 16 and shares the same residence must register in their place. Any changes in the address or registered information must be reported to the resident registration section of the relevant ward office in person or by attorney, as follows:

National Health Insurance (NHI)

All foreign nationals residing in Japan fot more than one year must join a Japanese public medical insurance program. Students who will stay in Japan less than one year, but have “college student” resident status are also eligible to join the program. Once enrolled, subscribers will receive a health insurance card. When they receive treatment at any healthcare provider that accepts NHI, they will only need to pay 30% of the expenses when presenting the insurance card at the reception desk before treatment.

Enrollment

Alien registration card and passport are basically necessary, but each local ward office may have its own requirements. Please check with your local office before you apply.

Payment

The annual premium based on the subscriber's previous year's income in Japan. Annual premium is paid in ten monthly installments from June to March. You may choose automatic withdrawal from a bank or a post office account (application necessary) or you can pay with a payment slip at a bank, post office or city or ward office.

Moving

If you change address, you must notify the city or ward office within 2 weeks.

Leaving Japan

Costs will need to be adjusted when canceling the policy. The NHI card must be returned to the city or ward office after the departure date is set. The office will modify the NHI expiration date so that it covers you until departure.

When a child is born in Japan

If a child is born during your stay in Japan, please make sure to conduct the following procedures:

Required Procedures

Register the birth by taking the birth certificate(Shussei Shomei-sho) issued by the hospital and the Mother-Child Health Handbook (Boshi-techo) to your city or ward office. A birth registration receipt (Shussei Todoke Juri-Shomei sho) will be issued.

The parent(s) living with the child must apply for the child's Certificate of Alien Registration with the receipt at the city or ward office within 14 days after the birth of the child.

Apply for the Certificate of Eligibility status "Dependent" for the child to stay in Japan longer than 60 days, by taking the receipt, passports and Alien Registration certificate of both parents, and Alien Registration certificate of the child, to the Immigration Bureau within 30 days after birth.

After receiving the Certificate of Eligibility (Zairyuu Shikaku Nintei-Shomei sho), have the status changereflected on the child's Alien Registration certificate at the city or ward office.

*Also, contact your home country's embassy/consulate in Japan to obtain a passport for the child, and add the child to your health insurance policy as soon as possible.

Subsidy for Households with Children

[ Lump Sum Birth Allowance (Shussan ichiji kin) ]

Japanese National/Employees' Health Insurance subscribers are eligible to receive this allowance. (Application is necessary)

[ Check-Ups and Vaccinations (free) ]

The city or ward office will send guidance about medical check-ups and vaccinations to families that registered the birth of their child. Please go to the appointed hospitals or venue.

[ Child-Care Allowance (Jido Teate) ]

Families registered as foreign residents with children who are 12 years old and or under are eligible to receive this allowance for each child from the city or ward office.(Application is necessary, and there are certain limitations to this allowance.) [ Discounted Medical Treatment for Children ]

Japanese National/Employees' Health Insurance subscribers are eligible for discounts on medical expenses for hospitalization and check-ups for children. For further details, please contact your city or ward office.
